The horizontal orientation is the point: sitting sideways on the belt, the pouch opens from the top like a dump pouch, so tools go in and out without unmounting anything. The small pull handle is big enough to grab but not big enough to catch on something during an operation, and the robust zipper won't break or jam when it's dirty — because it will get dirty.
